    Control Strategies and Design to Range in Light Railway Systems

    The thesis deals with different control strategies and a design method to improve energy efficiency and reliability in light railway transportation systems. The possibility of use Supercapacitors Energy Storage System (SESS) in light railway systems is explored, by evaluating the suitability of on-board application for a Prototype Railway Vehicle, with the introduction of a methodology for Design to Range in catenary free operations in order to fill gaps in power supply. Furthermore, a stationary configuration of SESS in light railway system is also investigated by means of a demonstrator of a Metro rail System set in Hitachi Rail Italy test room, carrying out a control strategy for energy flows management in case of non receptive DC grid. Further investigations has regarded the introduction of two sensorless control strategies for two different railway traction architectures with IM and PMSM drives, focusing the attention on energetic and dynamic performance in different specific operating conditions required to the railway traction drives. Numerical and experimental results are obtained and discussed in different operating conditions, for real case studies, showing the feasibility and the fulfillment of the mission for the different solutions proposed

    A network analysis on cloud gaming: Stadia, GeForce Now and PSNow

    Cloud gaming is a new class of services that promises to revolutionize the videogame market. It allows the user to play a videogame with basic equipment while using a remote server for the actual execution. The multimedia content is streamed through the network from the server to the user. This service requires low latency and a large bandwidth to work properly with low response time and high-definition video. Three of the leading tech companies, (Google, Sony and NVIDIA) entered this market with their own products, and others, like Microsoft and Amazon, are planning to launch their own platforms in the near future. However, these companies released so far little information about their cloud gaming operation and how they utilize the network. In this work, we study these new cloud gaming services from the network point of view. We collect more than 200 packet traces under different application settings and network conditions for 3 cloud gaming services, namely Stadia from Google, GeForce Now from NVIDIA and PS Now from Sony. We analyze the employed protocols and the workload they impose on the network. We find that GeForce Now and Stadia use the RTP protocol to stream the multimedia content, with the latter relying on the standard WebRTC APIs. They result in bandwidth-hungry and consume up to 45 Mbit/s, depending on the network and video quality. PS Now instead uses only undocumented protocols and never exceeds 13 Mbit/s

    Alexithymia and Suicide Risk in Psychiatric Disorders: A Mini-Review.

    It is well known that alexithymic individuals may show significantly higher levels of anxiety, depression, and psychological suffering than non-alexithymics. There is an increasing evidence that alexithymia may be considered a risk factor for suicide, even simply increasing the risk of development of depressive symptoms or per se. Therefore, the purpose of this narrative mini-review was to elucidate a possible relationship between alexithymia and suicide risk. The majority of reviewed studies pointed out a relationship between alexithymia and an increased suicide risk. In several studies, this relationship was mediated by depressive symptoms. In conclusion, the importance of alexithymia screening in everyday clinical practice and the evaluation of clinical correlates of alexithymic traits should be integral parts of all disease management programs and, especially, of suicide prevention plans and interventions. However, limitations of studies are discussed and must be considered

    Almond, Hazelnut, and Pistachio Skin: An Opportunity for Nutraceuticals

    Nuts are dry, single-seeded fruits, with a combination of beneficial compounds that aid in disease prevention and treatment. This review aims to summarize the antioxidant components and the nutraceutical properties and applications of hazelnut, almond, and pistachio skins, as well as discuss their ability to prevent and treat specific diseases based on in vitro and in vivo studies. The search strategy included searching PubMed database and Google Scholar for relevant articles published in English. Research articles focusing on hazelnut, pistachio, and almond were included. The nut skin extracts were considered and other by-products were excluded from this search. Pistachio and almond skin hydroalcoholic extracts have antibacterial effects and decrease the risk of liver cancer by eliminating reactive oxygen species. Moreover, hazelnut skin can lower plasma against low-density lipoprotein-cholesterol, thus reducing the risk of colon cancer, and its polyphenolic extract can also decrease the formation of advanced glycation end products in vitro with multidimensional effects. Overall, hazelnut, pistachio, and almond skins are a great source of antioxidants, making them suitable for nutraceuticals’ development

    Atypical Antipsychotics in the Treatment of Acute Bipolar Depression with Mixed Features: A Systematic Review and Exploratory Meta-Analysis of Placebo-Controlled Clinical Trials

    Evidence supporting the use of second generation antipsychotics (SGAs) in the treatment of acute depression with mixed features (MFs) associated with bipolar disorder (BD) is scarce and equivocal. Therefore, we conducted a systematic review and preliminary meta-analysis investigating SGAs in the treatment of acute BD depression with MFs. Two authors independently searched major electronic databases from 1990 until September 2015 for randomized (placebo-) controlled trials (RCTs) or open-label clinical trials investigating the efficacy of SGAs in the treatment of acute bipolar depression with MFs. A random-effect meta-analysis calculating the standardized mean difference (SMD) between SGA and placebo for the mean baseline to endpoint change in depression as well as manic symptoms score was computed based on 95% confidence intervals (CI). Six RCTs and one open-label placebo-controlled studies (including post-hoc reports) representing 1023 patients were included. Participants received either ziprasidone, olanzapine, lurasidone, quetiapine or asenapine for an average of 6.5 weeks across the included studies. Meta-analysis with Duval and Tweedie adjustment for publication bias demonstrated that SGA resulted in significant improvements of (hypo-)manic symptoms of bipolar mixed depression as assessed by the means of the total scores of the Young Mania Rating Scale (YMRS) (SMD −0.74, 95% CI −1.20 to −0.28, n SGA = 907, control = 652). Meta-analysis demonstrated that participants in receipt of SGA (n = 979) experienced a large improvement in the Montgomery–Åsberg Depression Rating Scale (MADRS) scores (SMD −1.08, 95% CI −1.35 to −0.81, p < 0.001) vs. placebo (n = 678). Publication and measurement biases and relative paucity of studies. Overall, SGAs appear to offer favorable improvements in MADRS and YMRS scores vs. placebo. Nevertheless, given the preliminary nature of the present report, additional original studies are required to allow more reliable and clinically definitive conclusions

    Spontaneous vertebral aspergillosis, the state of art: a systematic literature review

    Objective: Vertebral aspergillosis are quite rare conditions, often misdiagnosed, that requires long-term antibiotic therapy and, sometimes, surgical treatments. The present investigations was aimed to investigate epidemiology, clinical-radiological aspects, treatment protocols, and outcomes of Aspergillus-mediated vertebral osteomyelitis. Methods: A systematic review of the pertinent English Literature according to the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines was performed. The research was conducted on Cochrane library, MEDLINE, PubMed and Scopus using as search-terms “Aspergillus”, “vertebral osteomyelitis”, “spondylodiscitis”, “spine infection”. A case of vertebral apsergillosis conservatively managed was also reported. Results: Eighty-nine articles were included in our systematic review. Including the reported case, our analysis covered 112 cases of vertebral aspergillosis. Aspergillus fumigatus was isolated in 68 cases (61.2%), Aspergillus flavus in 14 (12.6%), Aspergillus terreus in 4 (3.6%), Aspergillus nidulans in 2 (1.8%). Seventy-three patients (65.7%) completely recovered at last follow-up evaluation; in 7 (6.3%) patients radiological signs of chronic infection were reported, whereas 32 (28.8%) patients died during the follow-up. Conclusion: This systematic review summarized the state of the art on vertebral aspergillosis, retrieving data on clinical features, diagnostic criteria and current limitations, treatment alternatives and their outcomes

    Cotard’s Syndrome: Clinical Case Presentation and Literature Review

    In 1880 French neurologist Jules Cotard described a condition characterized by delusion of negation (nihilistic delusion) in a melancholia context. Recently, there has been a resurgence of interest in Cotard’s syndrome. The most prominent symptoms of Cotard’s Syndrome are depressive mood, nihilistic delusions concerning one’s own body and one’s own existence, delusions of guilt, immortality and hypochondria. The aim of the present paper is to review literature evidences concerning Cotard’s syndrome and to describe a clinical case keeping in the background the recent trends on its psychopathological implications. In the clinical study, the following sequence of stages emerged: the dissociative side, expressed as a loss of body-mind cohesion; the ‘mixed’ mood disorder, with depressive-manic episodes, and a persecutory background, all coexisting in the anguish of the idea of a body falling apart, the anguish of a descent towards the abyss of melancholia and/or an ascent to unlimited euphoria, characteristic of an “uncommon alarm” for loss of Self cohesion

    Comparison between Capillary and Serum Lactate Levels in Predicting Short-Term Mortality of Septic Patients at the Emergency Department

    Sepsis is a time-dependent and life-threating condition related to macro- and micro-circulatory impairment leading to anaerobic metabolism and lactate increase. We assessed the prognostic accuracy of capillary lactates (CLs) vs. serum ones (SLs) on 48-h and 7-day mortality in patients with suspected sepsis. This observational, prospective, single-centre study was conducted between October 2021 and May 2022. Inclusion criteria were: (i) suspect of infection; (ii) qSOFA ≄ 2; (iii) age ≄ 18 years; (iv) signed informed consent. CLs were assessed with LactateProTM2Âź. 203 patients were included: 19 (9.3%) died within 48 h from admission to the Emergency Department, while 28 (13.8%) within 7 days. Patients deceased within 48 h (vs. survived) had higher CLs (19.3 vs. 5 mmol/L, p &lt; 0.001) and SLs (6.5 vs. 1.1 mmol/L, p = 0.001). The best CLs predictive cut-off for 48-h mortality was 16.8 mmol/L (72.22% sensitivity, 94.02% specificity). Patients within 7 days had higher CLs (11.5 vs. 5 mmol/L, p = 0.020) than SLs (2.75 vs. 1.1 mmol/L, p &lt; 0.001). The multivariate analysis confirmed CLs and SLs as independent predictors of 48-h and 7-day mortality. CLs can be a reliable tool for their inexpensiveness, rapidity and reliability in identifying septic patients at high risk of short-term mortality
